Expanded In-Home Services for the Elderly Program (EISEP) EISEP provides case management, in-home care, ancillary, and respite services to older adults who are ineligible to receive services through other government-funded programs. Case Managers meet with Nassau County residents 60 years or older and their family to first complete a comprehensive assessment, which examines existing needs for the senior as well as what barriers may be preventing them from remaining independently in their home. Upon determining what services are needed, such as non-medical in-home services, additional case management, non-institutional respite, home-delivered meals, ancillary services, emergency response systems, and more, the Case Managers work with the senior and family to develop a care plan and then implement the appropriate services. EISEP participants are eligible for the program regardless of income and are required to cost-share according to a sliding scale reflecting their income as well as the cost of services they receive, ranging from no-cost to full-cost.

Expanded In-Home Services for the Elderly Program (EISEP) This program connects Nassau County seniors with services through government agencies, parishes, and community-based organizations. A case manager works with each senior to assess needs and develop and implement a service plan, which might include home care, transportation, state and local entitlements, and other needed services.

Meals-on-Wheels Every weekday, the Meals-On-Wheels program delivers hot, nutritious meals to the homebound in Nassau and Suffolk counties. In Nassau, seniors may also participate in the frozen meal program, which provides meals five days per week. Meals-on-Wheels case managers can assist the seniors in accessing other services they might need.
Eligibility and Fees
Seniors must be 60 years of age or older to participate in any senior services programs. Meals-On-Wheels are available to seniors who are homebound and unable to prepare meals. A cost share may apply to EISEP services which is determined by a case manager during the assessment process. There are small suggested donations for Meals-On-Wheels and the senior centers. No one, however, is refused these services because of an inability to pay.

Nutrition Help for Seniors

The Commodity Supplemental Food Program (CSFP) offers free, nutritious foods to adults aged 60 years of age and older. Approximately 36,000 older adults participate in the Commodity Supplemental Food Program monthly. During the summer months, participants also receive additional benefits to purchase fresh fruits and vegetables at farmers’ markets through the Senior Farmers’ Market Nutrition Program.
