Housing assistance for low-income seniors

Expanded In-Home Services for the Elderly Program (EISEP) This program connects Nassau County seniors with services through government agencies, parishes, and community-based organizations. A case manager works with each senior to assess needs and develop and implement a service plan, which might include home care, transportation, state and local entitlements, and other needed services.

Meals-on-Wheels Every weekday, the Meals-On-Wheels program delivers hot, nutritious meals to the homebound in Nassau and Suffolk counties. In Nassau, seniors may also participate in the frozen meal program, which provides meals five days per week. Meals-on-Wheels case managers can assist the seniors in accessing other services they might need.

Eligibility and Fees
Seniors must be 60 years of age or older to participate in any senior services programs. Meals-On-Wheels are available to seniors who are homebound and unable to prepare meals. A cost share may apply to EISEP services which is determined by a case manager during the assessment process. There are small suggested donations for Meals-On-Wheels and the senior centers. No one, however, is refused these services because of an inability to pay.

Expanded In-Home Services for the Elderly Program (EISEP) EISEP provides case management, in-home care, ancillary, and respite services to older adults who are ineligible to receive services through other government-funded programs. Case Managers meet with Nassau County residents 60 years or older and their family to first complete a comprehensive assessment, which examines existing needs for the senior as well as what barriers may be preventing them from remaining independently in their home. Upon determining what services are needed, such as non-medical in-home services, additional case management, non-institutional respite, home-delivered meals, ancillary services, emergency response systems, and more, the Case Managers work with the senior and family to develop a care plan and then implement the appropriate services. EISEP participants are eligible for the program regardless of income and are required to cost-share according to a sliding scale reflecting their income as well as the cost of services they receive, ranging from no-cost to full-cost.

The elder abuse enhanced multidisciplinary team, or E-MDT, is made up of professionals from various fields brought together to discuss complex cases of elder abuse and create action plans to offer resources and services to the victim through a collaborative approach to elder justice.

Elder Care Resource Center: exists to be the community resource for support, answers and expertise in regard to advance planning and informed decision making about present and future long term health care. The Resource Center offers over 200 books and brochures, a website with over 2,000 links

FCA provides coordination of the Nassau County Elder Abuse Enhanced Multidisciplinary Team. E-MDTs are made up of professionals with expertise in senior care and abuse who review and intervene in complex cases of elder abuse. The Nassau County E-MDT helps victims residing in Nassau County who are 60 years of age or older through a collaborative, multi-agency approach to putting a stop to the abuse, as well as providing services to return the victim to a state of stability and safety.

Elder Care Options Assistance so Seniors may remain in their Home if it is their wish. Antique & Thrift Shop called the Trading Post, on site, funds LIFEC so they can continue to assist those who don’t have finances to utilize.

Rental/Tenant Counseling for Housing stability

Information on services available to any non-parent caregiver, including grandparents, relatives, and even non-relatives who provide full-time care for children in their home.

List of non-subsidized housing in Suffolk

The Commodity Supplemental Food Program (CSFP) offers free, nutritious foods to adults aged 60 years of age and older. Approximately 36,000 older adults participate in the Commodity Supplemental Food Program monthly. During the summer months, participants also receive additional benefits to purchase fresh fruits and vegetables at farmers’ markets through the Senior Farmers’ Market Nutrition Program.

This is for a Senior Wish which can come in many forms from a Simple Wish (i.e. Wheelchair) to Celebrating a “Life” Wish (Visiting Hometown one more time etc.). 

Serves low-income seniors, generally defined as individuals who are at least 60 years old and who have household incomes of not more than 185% of the federal poverty income guidelines.
