A voluntary developmental evaluation and services program offered by New York State and administered locally by Nassau County Department of Health Office for Children with Special Needs.
Eligibility: children must be under 3 years of ago and have a confirmed disability or established developmental delay, as defined by NYS, in one or more of the following areas of development: physical, cognitive, communication, social-emotional, and/or adaptive. Evaluations are conducted to determine eligibility.
Program Includes: identification and referral of children at risk or suspected of disability by primary referral sources, periodic developmental screening and tracking of at risk children, service coordination for eligible children and families, a multidisciplinary evaluation provided at no cost to parents to determine eligibility, Individualized Family Service Plans (IFSPs) for eligible children and families, provision of early intervention services in the IFSP at no cost to parents, and delivery of services in natural settings in the community where children of the same age are typically found, to the maximum extent appropriate.

School-Age and Early Childhood Family and Community Engagement (FACE) Centers help young New Yorkers with disabilities who are 0 to 21 years old. Help is also available for their families and the professionals who support them.
FACE Centers offer many services including:
• Guidance to help children with disabilities and their parents navigate early childhood.
• Teaching special education students to advocate for themselves in school. This includes teaching them how to participate meaningfully in their Individualized Education Program (IEP) meetings.
• Teaching parents and special education students about the college admissions process.

Crisis Retreat Program: short-term resitdent program with special supports for adults experiencing a mental health crisis. Provides 24-hour supervision in a warm, safe environment. These individuals do not need hospitalization. Residencts may stay from 1 - 30 days. Permanent Housing: Supportive Housing funded through OMH. Serves residents age 18 and over, who have psychiatric disabilities and can live independently with minimal support. Residents pay 30% of their income towards rent and utilities. Mobile Residential Support Team (MRST): Provides intensive support resources to people who live in Nassau County Supportive Housing. Individualized Residential Alternative (IRA) Programs: These programs provide services to people with mild to moderate intellectual disabilities. These individuals can live with minimal supervision and exhibit a high level of independence. One full-time residential coach works at each home providing oversight and guidance to the residents. Residents are taught a variety of skills, including money management, cooking, travel training, and home management.
Supervised IRA Programs: Residents are given an opportunity to experience a family environment while receiving the specialized care they require. Direct support professionals provide assistance in daily living and independent living skills. Support and training are provided in cooking, self-care, and grooming.
Respite IRA Programs: Provide temporary home-like environment for individuals that live at home with their families and need some time away. It is clinically designed to support people experiencing a housing crisis and who are at risk of personal safety.
Intermediate Care Facilities (ICF): These homes are designed to accommodate the needs of the intellectual and are designed to provide maximum safety. Needs are assessed and an individual plan is developed upon admission. The goal is to provide a genuine sense of belonging and to nurture physical, psychological, and emotional growth.

Homeless Individuals/Families with HIV/AIDS: supportive housing for homeless individuals/families with HIV/AIDS, and housing financial assistance for people living with HIV/AIDS.
Homeless Individuals/Families with Chronic Illnesses and/or Disabilities: supportive housing for homeless individuals/families with disabilities, and supportive housing rental subsidies for homeless health home members.
Rapid Re-Housing: provides a rental subsidy, housing access, and the resources and support needed to help families exit homelessness and transition to permanent housing. Services typically last between 6 to 24 months.

Community Residences – Residents are provided with structed daily living support that enhances their growth and development. Residents are encouraged, guided and supported as they work, travel and interface within their communities. All activities and goals are designed to create an enviable life for each person supported.
Supportive Individualized Residential Alternative Program – Provides independent apartment living for adults. Residents either share or live alone in an apartment in the community and are provided support staff based on their individual needs. Residents gain the necessary skills to live, work and interact within their communities as independently as possible.
Community Habilitation Program – provides habilitation services within a person’s private home. The number of hours provided by staff varies based on each person’s needs. One of the main goals of this program is to increase a person’s independence while also providing support to both the person supported and their family. The program encourage people to strive to live independently in the community with minimal staff support.
